Completed commercial applications should include a $95.00 nonrefundable processing fee in the form of Electronic Payment, copy of your Social Security Card or IRS Form, ID/Drivers License and proof of income (recent bank statement or previous year federal tax return). Applications are considered incomplete until all necessary documentation is supplied.
